Record / Document

Access / Inform

How This Works

An individual's electronic health record is accessed by health care providers to inform their care decisions. Health care providers formulate care delivery for the individual, and record information such as health care encounters, health care plans, medication, etc. in the electronic health record.

Record / Formulate

Access / Inform

How This Works

Health care providers administer encounters based on the disclosed information. Encounters are events where services or products are provided to assess, maintain or improve an individual’s health.

Disclose

Administer

How This Works

Issues and activities trigger encounters such as a visit to a family doctor or an emergency room, a massage, consumption of a medication, an operation or procedure. Encounters provide feedback such as diagnoses, prescription of drugs, clinical procedures, that help improve the individual's health.

Issues & Activity

Feedback

How This Works

Individuals document and record health activities in the electronic health record. They access the record to stay informed and proactively manage their health care.

How This Works

Access points, such as lab information systems, portals, hospital information systems, or pharmacy systems, deliver end user requests to the HIAL. Responses received from EHR resources are assembled by the HIAL and delivered to access points. Data from access points is delivered to the HIAL for storing in EHR resources.
